“Bird on the Wire”

The so-called “nutcracker phenomenon” is depicted in a patient with Crohn’s disease. Weight loss resulted in the left renal vein (here resembling a bird) being pinched between the aorta and superior mesenteric artery (SMA) with secondary engorgement of the left renal vein and an adjacent lumbar vein. This case was symptomatic due to an associated dilated gonadal vein and left-sided varicocele.
